File notes of telephone discussions

An agent put forward a file note of a record of telephone discussions that had allegedly taken place with the client. The client denied having spoken to the agent on a number of the dates recorded in the file note.

FDS undertook a series of detailed examinations that revealed that the file note had actually been written at least a year after the purported dates of the telephone discussions.
